Family Service Care Coordinator
Full-Time (Wakefield, RI)
General Responsibility: The Family Service Care Coordinator (FSCC) is responsible for assessing family needs and strengths, identifying appropriate services, enhancing supports, and ensuring that referrals are made to appropriately matched services and supports. The FSCC will work with families with children and youth who are at risk for abuse and neglect, who have serious emotional disturbance (SED) or a developmental disability (DD), who have juvenile justice corrections involvement in the home and/or school setting or are at risk for such involvement. The FSCC will be responsible for the functions of the Wraparound Facilitator, including family engagement, facilitating the wrap process, writing and revising the family care plan, scheduling and facilitating care plan team meetings. The position requires rotation through multiple service sites in the Washington/Kent Counties region.
Qualification: Bachelor’s Degree in social work or related field; training and/or experience with system of care principles including; home visiting, early childhood, juvenile justice, child welfare, or other appropriate background. Bilingual skills and experience in Washington/Kent Counties region and services preferred.
