Role:
This employee works with individual families approved for the Permanent Supportive Housing Program (PSH). This includes developing an individualized housing and service plan, including a path to permanent housing stability.
Essential Functions & Responsibilities:
30% Establish a working interaction with individuals/families who are enrolled in PSH for the purpose of providing assistance with attaining/maintaining adequate housing and access to needed services.
15% Provide information and referral to community resources, including mental health, AODA, medical, educational, financial services, etc. based on assessment. Follow up with clients to ensure continued access to services. Assist with addressing barriers to accessing resources as needed.
30% Maintain updated case records/files. Case records are to be up to date and available for generating reports, audits and grant writing. Enter all necessary client data into HMIS and EmpowOR within the grant required time frame. Maintain confidentiality in all aspects of the position.
15% Complete monthly PSH File Reviews with the Housing Administrative Director to ensure the file in in compliance with PSH Grant program requirements.
5% Build and maintain positive relationships with housing providers and community partners.
5% Other duties as assigned

Knowledge, Skills and Abilities:
Experience One year to three years of similar or related experience.
Education
(1) A two-year college degree or
(2) completion of a specialized course of study at a business or trade school or
(3) completion of specialized training courses conducted by equipment vendors or
(4) job specific skills acquired through on-the-job training or apprenticeship program.
Interpersonal Skills:
A significant level of trust and diplomacy is required, in addition to normal courtesy and tact. Work involves extensive personal contact with others and is usually of a personal or sensitive nature. Work may involve motivating or influencing others. Outside contacts become important and fostering sound relationships with other entities (companies and/or individuals) becomes necessary and often requires the ability to influence and/or sell ideas or services to others.
