Social Worker, LMSW

Job Summary
We are seeking a compassionate and dedicated Social Worker to join our Home Health Agency team. The ideal candidate will possess a strong commitment to improving the lives of individuals and families through effective support and intervention. This role involves working with diverse populations, providing guidance, and connecting clients with necessary resources. The Social Worker will play a critical role in crisis management, behavioral health support, and workforce development initiatives.

Responsibilities

• Conduct assessments to identify clients' needs and develop personalized care plans.
• Utilize motivational interviewing techniques to encourage positive behavioral changes.
• Facilitate access to behavioral health resources and addiction counseling as needed.
• Collaborate with community organizations to enhance service delivery and client outcomes.
• Maintain accurate documentation of client interactions, progress, and outcomes.
• Participate in ongoing professional development to stay current with best practices in social work.

Qualifications

• Bachelor’s or Master’s degree in Social Work or related field.
• Proven experience in social work, particularly in crisis management and behavioral health settings.
• Strong skills in mentoring, motivational interviewing, and addiction counseling preferred.
• Knowledge of workforce development strategies is a plus.
• Excellent communication skills, both verbal and written.
• Ability to work collaboratively with multidisciplinary teams and community partners.
• Compassionate approach with a strong desire to make a positive impact on clients' lives.
• Previous home health experience preferred
