We are seeking a compassionate and skilled Registered Nurse to join our hospice team. The ideal candidate will have experience in both outpatient and hospice care settings and will be comfortable providing comprehensive nursing care to patients in their homes. Your expertise will help patients maintain their independence and well-being while receiving care in the comfort of their homes.

Key Responsibilities:
- Conduct comprehensive patient assessments to determine their health needs and develop personalized care plans
- Provide skilled nursing care, including medication administration, wound care, and other medical treatments, following established protocols.
- Collaborate with the interdisciplinary team to ensure a holistic approach to patient care
- Educate patients and their families on health management, disease prevention, and self-care techniques
- Monitor patients' conditions and adjust care plans as necessary to achieve optimal outcomes
- Maintain accurate and up-to-date patient records and documentation
Qualifications
- Active Vocational Nursing License
- Previous Experience in Hospice or other Medical Fields
- Familiarity with Medical Software and Equipment
- Updated CPR/BLS Certificate
- Valid California Driver's License

Pay: $100.00 - $120.00 per hour
