Job Overview
Nurse Practitioner for Mobile Health Clinic
This position is responsible for providing primary care services, including assessing, diagnosing, prescribing, treating, and educating patients for the Mobile Outreach Service. Works closely with physicians and other clinicians in a team approach to patient care.
DUTIES AND RESPONSIBILITIES:
- Provides diagnostic and assessment services. Obtains screens, and evaluates health history and performs basic physical assessment of patient in accordance with established protocols, procedures, and practices. Orders laboratory tests and radiology exams when appropriate.
- Provides therapeutic services. Collaborates with physician and other healthcare providers for confirmation of therapeutic services needed to prepare a comprehensive patient care plan.
- Assesses and manages specific illnesses and injuries established protocols.
- Participates in follow up on patient which may include telephone calls.
- Advises and counsels patients.
- Refers patients for specialty consultations as appropriate and in accordance with medical protocols.
- Identifies community health resources and guides patients and/or significant other in their use.
- Relays preadmission information for hospital care.
- Performs charting duties as required. Prepares and maintains required records, reports, statistics, and files.
- Reviews schedules, and sets up appointments.
- Gives specific patient care directions to clinic staff.
- Assumes responsibility through a team approach for effective management and efficient implementation of patient care standards .
- Participates in a team governance approach including peer review and case management review with other nurse practitioners.
- Initiates and/or participates in community outreach activities.
- Provides general supervision of medical assistants and assists in evaluation of performance.
- Participates as a faculty advisor for nursing students providing teaching, consultations, and academic support.
- Performs other duties as required.
License and Certification:
State Licensure as a Registered Nurse in Tennessee
Certification as Nurse Practitioner
State Licensure as Advance Practice Nurse in Tennessee
Job Related Experience
Two (2) years clinical nursing experience required. Committment to a Continuous Quality improvement philosophy and process. Strong interpersonal and communication skills. Knowledge of age specific learning principles. Knowledge of CDC Standards of cleanliness and safety, JCAHO Standards, and other state and federal regulatory agency requirements.

Pay: $9,000.00 - $10,833.00 per month
