What are the responsibilities and job description for the ANCHOR/Crisis Intervention position at Tri-Valley, Inc.?
The ANCHOR/Crisis Worker manages the ANCHOR program including casework for the ANCHOR clients. Provides back up for the Crisis Program. Determines the need for protective intervention and the delivery or arrangement of services for self-neglecting elders. Carries case load at program standard.
Responsibilities:
Anchor
- Primary responsibility to accept ANCHOR referrals from supervisor on appropriate cases requiring intensive and extensive casework and crisis referrals as needed.
- Completes a minimum of one home visit and two phone calls per month for all ANCHOR clients.
- Maintain monthly contact with the fiscal department to review billing/authorizations to determine if any adjustments are needed.
- Attend the monthly Shared Cases meeting with SHC and CSI departments to review ANCHOR/Crisis enrollment and potential clients for the programs.
- Conducts an assessment of each client referred and establishes an appropriate service plan.
- Maintains records regarding the case plan including physio-psycho-social characteristics and disposition of said clients.
- Provides consultation service to other Agency staff when appropriate.
- For ANCHOR clients document in SAMS to include Journal notes, A & Rs, Service Deliveries and Care Enrollments.
- Provide backup and assistance to other Crisis Intervention workers when directed by Supervisor.
- Prepares appropriate statistics and reports to document program activities.
- Performs miscellaneous duties and assignments as directed by immediate supervisor.
- Maintains work area in a neat and professional manner.
Crisis
- Maintains Crisis client records in WORD and completes one home visit per month.
- Utilizes community professionals who can act as consultants in developing case plans for clients in crisis.
