What are the responsibilities and job description for the State Home Care Supervisor position at Tri-Valley, Inc.?
The basic function of the Supervisor for State Home Care is to direct supervision of the daily requirements of case management staff. Assists the Director in the performance of his/her duties.
Responsibilities:
- Reviews cases to evaluate service plan, service delivery and eligibility for specialized programs.
- Approves applicants’ need for service, approves the plan and level of care within authorized limits and guidelines and ensures consumer choice is reviewed.
- Reviews and approves openings, closing, Annual Reassessments and ensures that changes are updated in the computer, paperwork uploaded into file attachments and the electronic file room, and all resources/benefits have been reviewed with the consumer.
- Assigns referrals to staff.
- Conducts field visits with each Case Manager yearly.
- Ensures staff maintains contact with clients per EOEA regulations.
- Communicate with Fiscal Department regarding billing issues and or overdue consumer bills.
- Participates in interviews and effectively recommends to the Program Director the hiring of all personnel reporting directly to him/her.
- Indoctrinates new staff members with regard to day-to-day management operational practices, policies and procedures.
- Provides training to newly hired Care Managers utilizing Tri-Valley’s State Home Care Training Manual.
- Assigns, directs, and oversees the work of all staff under his/her direct supervision through regularly scheduled supervision sessions.
- Ensures all duties assigned to staff are performed accurately and within program specifications.
- Involves staff in the development, clarification and continued upgrading of operational procedure and policy by soliciting comments, suggestions, participating in pilot groups and updating business practices.
- Effectively recommends transfer, discipline, suspension or termination of staff under his/her supervision.
- Reviews “At Risk” clients at quarterly intervals and as needed due to weather emergencies.
- Fulfills executive/supervisory responsibilities as specified in Supervisor Performance Evaluation.
Essential Functions/Qualifications:
- BA/BS in relevant field.
- Three years case management experience in relevant organization
- Thorough understanding of Federal and state employment laws (preferred).
- Familiarity with state wide health care network (preferred).
- Establish and analyze data metrics for contract and regulatory compliance.
