Job Summary: We are seeking a skilled and dedicated Home Health Admission and Re-Certification Nurse to join our team. The ideal candidate will demonstrate proficient skills using OASIS to admit, transfer, re-certify, resumption, and discharge home health patients in accordance with physician orders. This role involves coordinating delivery of patient care, maintaining compliance with agency policies and procedures, and adhering to CMS regulations.
Responsibilities/Essential Functions:
Perform initial home care patient admission and regularly re-evaluate patient needs and progress.
Initiate the plan of care under doctor’s orders.
Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for home care patients.
Observe, assess, and document symptoms; monitor reactions and patient progress.
Educate patients and caregivers on disease processes, medications, plan of care, and individualized treatment plans.
Educate patients and caregivers on techniques for in-home health care.
Notify the physician and other personnel of patient condition changes.
Monitor the client's condition and promote sound preventive practices.
Evaluate the effectiveness of nursing services to the patient and family continuously.
Perform skills outlined in the agency’s approved policy and procedure manual.
Case conferences with clinicians providing care to ensure coordination of care.
Update clinical records according to policy and procedures.
Update knowledge and skills by attending in-service, continuing education, seminars, and self-study programs annually.
Adhere to CMS regulations.
Qualifications/Educational Requirements:
Registered Nurse (R.N.) with a current license.
Minimum of one year of nursing experience as an R.N.
Ability to work positively and favorably with patients, families, and staff.
Bi-lingual in any language added advantage
Knowledge of CMS guidelines.
Working knowledge of home health care principles and techniques.
Skillful in organization and time management.
Ability to contribute to the quality of care through constructive communication with nursing managers and staff.
Salary : $70 - $80
