About the Job
The Telephonic Care Manager oversees case management for MSK and URO patients, guiding them from initial assessment through recovery. In this remote role, the Care Manager collaborates with a multidisciplinary team to ensure continuity of care, optimize outcomes, and reduce unnecessary healthcare use.
Key Responsibilities
- Manage medical case management of MSK and URO patients, from assessment through recovery.
- Conduct telephonic assessments, care planning, and evaluations, focusing on value-based care principles.
- Coordinate care with providers, insurers, and other stakeholders for effective, timely interventions.
- Use TRIARQ’s digital tools to monitor patient progress, enabling proactive management and early risk identification.
- Collaborate with Care Coordinators on administrative tasks, focusing on clinical decision-making.
- Advocate for patient interests while maintaining cost-effective care strategies.
- Educate patients on self-management, adherence, and recovery pathways to empower healthcare involvement.
- Document and report patient progress, barriers, and outcomes to the healthcare team and payers as necessary.
- Ensure compliance with care protocols, patient engagement metrics, and industry standards.
Qualifications
- Current RN license in at least one state: Michigan, California, Florida, New York, Illinois, or Texas.
- Experience managing MSK or URO patients, ideally in value-based care.
- Certification in Case Management (CCM) or relevant certifications (CRRN, CDMS, CRC) preferred.
- Strong organizational skills with high autonomy.
- Proficiency in multiple systems, applications, and digital case management tools.
- Strong communication skills with a passion for patient education.
