JOB DESCRIPTION

ROLE AND RESPONSIBILITIES

Community Navigators (CNs) serve as role models, mentors, and advocates in building a sense of hope, improve social functioning, self-empowerment and increase overall quality of life. CN’s perform services involving recovery support, building connections to outside resources, milieu management, ensuring daily program schedule adherence, planning and implementation of recreational, therapeutic, and educational services.

Job Duties Include:

  • Facilitate individual and group therapeutic activities including coaching, milieu management, and case management.
  • Performs outreach to build relationships with participants using a trauma informed peer support model.
  • Completes intake interviews and assesses needs for medical, psycho-social, and psychiatric evaluations.
  • Documents all notes and participant interactions in EHR system and other monitoring systems as needed.
  • Reviews and helps implement therapist evaluations, treatment objectives, and plans . Helps set schedules, routines, and coordination of services.
  • Engages participants with intentional and skillful self-disclosure of personal experiences to help overcome challenges and build trust and rapport. Strengthens participants hope, empowerment, and positive action.
  • Offers crisis support, safety and validation and provides crisis resources as needed.
  • Provides feedback to the clinical team regarding participant’s level of motivation, participation in group and social activities and milieu interactions.
  • Upholds program rules, guidelines and policies and assists participants in following all rules, guidelines, and policies.
  • Assists with arranging transportation to and from appointments when necessary.
  • Helps to assesses, collaborate and coordinate participant’s discharge planning needs and readiness for discharge with referral sources and other members of the clinical team. Documents discharge plans and summary in EHR system and all other applicable monitoring systems.
  • Attends all required multidisciplinary staff meetings, referral source meetings, stakeholders and court system meetings.
  • Builds professional relationships and networks with community resources and service providers across systems for the populations served.
  • Obtains urine drug test specimens and breathalyzer tests.
  • Always maintains safety of the milieu. This includes awareness of client location and high-risk situations.
  • Maintain daily program and special events schedule.
  • Document shift summary reports and sends via appropriate channels.
  • Follow all requirements when dispensing and/or providing medication to participants.
  • Performs light janitorial duties.
  • Performs bed checks when applicable.
  • Other duties as assigned.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

EDUCATION: High school diploma or equivalent. Bachelor’s Degree in field related to counseling, therapy, social sciences preferred.

EXPERIENCE: Personal and/or professional experience. Work with treatment facilities and/or with addictions and mental health populations is preferred. Able to demonstrate strategic decision-making, problem solving skills and timely documentation.

LICENSURE/CERTIFICATION: Peer Coach or Peer Specialist Certification. Colorado certification for addiction counseling, CAT preferred.

Conflict Resolution skills. Rapport building Skills. Able to maintain emotional balance in stressful/difficult situations. Computer skills. Driving ability.
