What are the responsibilities and job description for the RN Home Health position at Tribute Home Health Care?
We are looking for a registered nurse to join our team. The RN will plan, organize and direct home care services and is experienced in nursing with emphasis on home health care. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Pay for this job ranges from $140.00-$175.00 per case assessed.
Patient Care Responsibilities:
- Completes an initial assessment of patient in home environment to determine home care needs.
- Provides a complete physical assessment and history of current and previous illness(es).
- Conduct thorough patient assessments and maintain accurate documentation.
- Utilize OASIS assessment and documentation.
- Provide nursing care through all phases of care taking into consideration age and developmental needs
- Collaborate and support plan of care as determined by the physician and healthcare team
- Treat patients accordingly as approved by the physician.
- Perform periodic re-evaluation/ re-certification of the patient as necessary and makes adjustments in the patient's treatment program.
- Provide treatment to patients per protocols
- Facilitate timely discharge based on goals and in accordance with client policies and procedures
- Educate and instructs the patient, patient's family and other staff as required.
- Counsel the patient and family in meeting nursing and related needs.
Qualifications:
- Valid RN License with experience in home health care
- OASIS experience preferred but not required
Pay: $140.00 - $175.00 per hour
