Salary : $43 / hr. - $45 / hr.
Job Summary :
Provide patients and their families, including vulnerable populations, including those with mental health related diagnosis; with the psychosocial support needed to cope with chronic, acute, or terminal illnesses.
Services address their social, emotional, and financial needs. Services provided include clinical assessment, crisis intervention, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community.
These services advise family caregivers, provide patient education and counseling, and make necessary referrals to community resources for continuity of care.
Assist family members in understanding, dealing with, and supporting the client or patient.
Collaborates with the discharge planning team to facilitate and arrange appropriate discharge needs of the client according to standards set by CIHQ and CMS.
Bachelor's degree in social work or related field required
