Full Time Pediatric LISW or LPCC
The TriHealth organization is recruiting for a licensed behavioral health professional to work with our West Chester and Mason pediatric teams. This role provides direct behavioral health services to patients and families, including consultation, assessment, counseling/psychotherapy, education, prevention, and referral coordination.
Key Responsibilities:
Clinical Service: Provide behavioral health services, including assessment, diagnosis, and treatment in an ambulatory care setting. Utilize evidence-based interventions such as motivational interviewing, problem-focused therapy, mindfulness, cognitive behavioral therapy, and acceptance and commitment therapy.
Consultation and Collaboration: Develop and maintain collaborative relationships with primary care providers, ambulatory practice teams, behavioral health team members, patients, and families. Provide behavioral health education to staff, patients, and families in individual and group settings. Collaborate with community mental health providers and support services to develop and provide a continuum of care for patients.
Case Management and Treatment Coordination: Work collaboratively with primary care teams to provide referrals, case management, and follow-up services for patients with behavioral health needs. Engage patients, initiate referrals, address barriers to care, and follow up. Familiarity with community resources to address patient psychosocial and behavioral health needs.
Documentation: Provide accurate and timely documentation in electronic health records to support the identification and treatment of patient psychosocial and/or behavioral health needs. Capture program evaluation and fidelity measures as directed.
Quality Improvement: Collaborate to monitor the behavioral health program and identify areas for improvement. Participate in meetings, quality improvement activities, continuing education, community education, and serve on committees or related activities as directed by supervisor.
May be asked to provide supervisory support to other team members, including professional guidance, assessment of supervisee's performance, approval of intervention plans, and assumption of responsibility for the welfare of supervisee's clients.
Requirements:
Master's Degree
Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Consultant (LPCC)
Generalist knowledge of behavioral health conditions and evidence-based treatment modalities to serve diverse groups of patients across the lifespan
Ability to work in a team-based integrated brief treatment model
Up to 1 year experience as a Professional Clinical Social Worker or Clinical Consultant
