Responsibilities:
- Develops and implements effective diabetes management plans for patients
- Provides personalized education and coaching to patients with diabetes and their families
- Works collaboratively with healthcare teams to ensure seamless care transitions
- Stays current with the latest research, guidelines, and best practices in diabetes care and education
- Promotes disease prevention and management strategies through community outreach and education
Requirements:
- Bachelor's degree in nursing, dietetics/nutrition, or a related field
- Certification as a CDCES by the Commission on Dietetic Registration or equivalent experience
- 2-3 years' experience in diabetes care and education or a related field
- Excellent communication, interpersonal, and problem-solving skills
- Ability to work effectively in a fast-paced environment
