Are you a skilled clinical pharmacist looking to make a direct impact on patient care in a hospital setting? We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our multidisciplinary team and play a crucial role in ensuring safe, effective, and individualized medication therapy for our patients.
As a Clinical Pharmacist, you will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. You’ll provide critical expertise in drug therapy management, medication reconciliation, and patient education, ensuring that our patients receive the highest standard of care.the Clinical Pharmacist collaborates with other health care providers to ensure appropriateness of all elements of pharmaceutical care and independently operates within policies and procedures to manage the department’s clinical and consultative services. The Clinical Pharmacist supervises the work of supportive team members to ensure accuracy and efficiency of distributive services. Additional responsibilities include, but are not limited to serving as a drug information resource for TriHealth providers, direct patient interaction, precepting of pharmacy residents and / or student pharmacists, and contribution to department projects or initiatives in collaboration with Pharmacy Leadership.
