Job Overview:
The Certified Diabetes Care and Education Specialist (CDCES) plays a crucial role in delivering comprehensive and seamless services that bridge the gap between clinical and self-management aspects of diabetes and cardiometabolic care. As an integral part of the interprofessional team, this specialist collaborates with patients to provide patient-centered care and education conducive to behavior change and improved quality of life.
Key Responsibilities:
- Delivers quality diabetes self-management education and medical nutrition therapy in individual and group settings based on assessed needs
- Utilizes appropriate teaching techniques sensitive to the learning preferences of patients with diabetes
- Manages virtual classes, including presentation of material, answering questions, and pre and post-class communication and documentation
- Completes comprehensive assessments for 1:1 patient visits, including emotional and behavioral health, interprets personal health data, and develops an individualized care plan based on patient-assessed needs and goals
- Effectively demonstrates ability to evaluate pertinent labs related to diabetes and associated comorbidities
- Collaborates with patients and providers to establish targets and individualized goals
- Demonstrates ability to manage a panel of patients, including initial assessment, development of care plans, and regular updates for providers
Working Conditions:
This position requires frequent standing, walking, and talking. The employee may occasionally sit, use hands to finger, handle, or feel objects, tools, or controls. Color vision is consistently required. The employee must occasionally lift and/or move up to 10 pounds and push/pull heavy equipment as needed. Climbing, stooping, kneeling, crouching, balancing, bending, reaching overhead, using foot pedals, and repetitive motion are rarely performed tasks.
