What We’re Looking For :

Trillium Health Resources is seeking a compassionate, dedicated, and highly organized DSS / Foster Care Manager to join our team. This community-based position is responsible for providing Tailored Care Management to children and youth within the DSS / Foster Care system who have complex physical, behavioral health, and social determinant needs. The DSS / Foster Care Manager will work closely with the Integrated Care Team and act as the primary liaison between DSS and other service providers to ensure holistic, whole-person care.

On a typical day, you might :

• Assist in identifying available services and resources.
• Provide care management to assigned members.
• Works cross-functionally with other Trillium departments.
• Facilitate referrals for health-related service.
• Provide education. communicates treatment updates and other member related activities.
• Document activities in the Trillium Connections care management platform.

Qualifications :

Education : Bachelor’s, Master’s, Other

Preferred education : Degree in Human Services

Experience :

Must meet QP status related to the MH / SU population :

Additionally, if serving members with LTSS needs, the care manager, must meet the minimum QP requirements defined above (A-D) and shall additionally have at a minimum two (2) years of prior LTSS and / or HCBS coordination, care delivery monitoring and care management experience. This experience may be concurrent with the years of experience required to become a Qualified Professional.

Preferred Experience : Working with child / youth members who are dually diagnosed with co-occurring disorders. Preferred work experience with DSS / Foster Youth Populations.

License / Certification : If Qualifying as an RN

Must have a valid driver’s license

Must reside in Trillium’s Mid State region (Anson, Guilford, Montgomery, Randolph and Richmond counties)
