What We’re Looking For:
Trillium Health Resources has a career opening for a Next Step Rehousing Specialist/Extender to join our Transition to Community Living (TCL) team. As a member of the team, you’ll be responsible for providing education and referrals/linking to community supports, as well as ensuring linkage to behavioral an physical health supports for individuals identified in the TCL populations who are in need of being rehoused in the community. Including collaboration with the Transition team Integrated Care. You will be engrained the in the community you are serving. The Next Step Rehousing Specialist/Extender works in collaboration with other team members to determine member’s need and completes required documentation and tasks in the Trilliums software platform as well as all state identified databases in accordance with designated timeframes.
On a typical day you might :
- Assist with rehousing members who have exited TCL housing and wish to continue to live independently within 90 days of exiting housing.
- Educate landlords/property managers/owners on TCL and the TCL Voucher subsidy.
- Provide technical assistance to landlords regarding specific members.
- Perform peer monitoring/reviews on three charts per month as a compliance and quality strategy.
- Adhere to all NCQA, EQR, and DOJ requirements

Qualifications:
Minimum Education:
High School or GED
Required Experience:
- A minimum of one (1) year paid experience in the Health and Human Services field, including experience working with any combination of the BH/SU/IDD population or, paid experience working directly with people experiencing serious and persistent mental illness, severe or prolonged substance use, intellectual or developmental disabilities. Duties such as experience teaching BH/SU/IDD population students, or working with them in institutional settings, performing general outreach with individuals, coordinating services/appointments, engaging in health promotion activities, linking members to community resources, and supporting a care team would be qualifying experience.
- Be a person with one (1) year live experience with a behavioral health condition with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system.
- Be a person with one (1) year lived experience with a behavioral health condition and a Certified Peer Support Specialist.
Relevant experience is defined as experience working with individuals with mental health, substance use and/or I/DD. Experience and knowledge of peer support and experience working with individuals who are transitioning from a facility to independent living.
Preferred Experience:
Experience in housing the TCL Special Needs population as well as knowledge of TCL voucher and Federal vouchers such as Section 8, HUD, Targeting and Key.

Must have a valid driver’s license
Must reside in North Carolina to be considered for remote status
