Personal Care Social Worker
CURRENT OPENING:
- Full-time
(8:00A-4:30P)
The Personal Care Social Work is to provide
quality social work services to individual residents while taking into consideration
their needs, desires, abilities, problems, and family situations. This position
will be performed in accordance with TANDEM LIVINGS’ Core Values, Mission
Statement, and regulations.
QUALIFICATIONS:
- B.S.W.,
B.S., or B.A. in Social Work or Behavioral Sciences required.
- At
least one (1) year of experience working with a geriatric population is
preferred.
- Basic
computer skills needed; Proficient with Microsoft Office, including Work and Outlook.
RESPONSIBILITIES INCLUDE:
- Orients and assists new residents in their adjustment to the
facility. Assigns a new resident ambassador and meets with new residents to
provide orientation and tour.
- Addresses various issues that inhibit the resident from
experiencing a reasonable level of quality of life - understanding level of care changes, illness, aging process, death of a loved
one, etc.
- Meets
with residents and/or families to assist with the Trillium Place Assistance or
SSI process. Starts application, reviews guidelines, and discusses start date.
Maintains contact with OOA, SSA.
- Coordinates
community services: Psychiatry and Psychotherapy Services, Hospice referrals,
and other services.
