Job Summary
The Care Manager plays a crucial role in supporting individuals and families in navigating complex systems to access necessary services and resources. This position involves assessing client needs, developing personalized care plans, and coordinating services across various sectors, including behavioral health, child welfare, and workforce development. The ideal candidate will demonstrate a commitment to empowering clients through effective advocacy and support.
Duties
- Conduct comprehensive assessments to identify client needs and strengths.
- Develop and implement individualized service plans that address specific goals.
- Coordinate with various service providers to ensure clients receive appropriate resources.
- Provide crisis intervention and support during critical situations.
- Offer mentoring and guidance to clients, fostering personal growth and self-sufficiency.
- Maintain accurate documentation of client interactions, progress, and outcomes.
- Facilitate access to programs such as Medicare, Section 8 housing, and addiction counseling.
- Utilize motivational interviewing techniques to encourage client engagement and commitment to their care plans.
- Collaborate with multidisciplinary teams to enhance service delivery and outcomes.
- Assures quality of care by adhering to Center of Excellence (COE) standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following standards of care and governing agency regulations.
Skills
- Strong understanding of workforce development principles.
- Knowledge of behavioral health practices and child welfare systems.
- Proficiency in crisis intervention strategies.
- Experience in addiction counseling is preferred.
- Background in social work or related fields is advantageous.
- Excellent communication skills, both verbal and written.
- Ability to build rapport with diverse populations and foster trust.

- Capacity for empathetic listening and problem-solving.
Minimum Education Requirements
- A bachelor’s degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice, or other related social sciences
OR
- Associates degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice, or other related social sciences with work experience in the recovery field
This position requires a dedicated professional who is passionate about making a positive impact in the lives of clients through effective case management practices.
Travel is required, and paid, to additional offices such as Latrobe, Monroeville, and Connellsville. Training is in our Butler office.

Pay: $16.00 - $23.00 per hour
