Trinity Care, a leading mental health and substance recovery agency, is seeking a compassionate and dedicated Mental Health Skill Building Specialist QMHP-A to join our team!
Under the direction of the Clinical Director and within the limitations of agency policy, you will provide training and support services to individuals aged 16 and older who are experiencing significant psychiatric diagnoses and functional limitations. Your role will be crucial in helping clients achieve and maintain stability in the community, supporting them as they navigate both mental health and substance recovery challenges in the least restrictive environment.
As a Mental Health Skill Building Specialist QMHP-A, you will collaborate with clients and their support systems to develop individualized treatment plans, conceptualize treatment approaches, and provide case management to promote long-term recovery. Your contributions will empower individuals on their path to greater independence, personal growth, and overall well-being.
Key Responsibilities:
- Provide Mental Health Skill Building services, including independent living skills, case management, and other related services to individuals with mental health and substance use challenges.
- Develop individualized treatment plans with specific goals, objectives, and discharge criteria to meet the needs of clients and their families.
- Conceptualize presenting issues related to mental health and substance recovery, and design appropriate interventions.
- Coordinate with collaterals (family, other healthcare providers, etc.) to address case issues and ensure integrated care.
- Participate in community collaborative meetings to help improve mental health and substance recovery services.
- Maintain accurate case documentation in line with program policies and third-party billing requirements.
- Travel within the greater Richmond area for client visits.
- Participate in weekly clinical staff supervision meetings with the Program Supervisor and other mental health support clinicians.
Qualifications:
Minimum Requirements:
- Associate's Degree and at least 2 years of human services experience OR
- Qualified paraprofessional mental health provider with sufficient training, education, and/or experience.
- A background in social work, psychology, or a related field is preferred.
- Experience in crisis intervention and patient care is essential.
Preferred Qualifications:
- Bachelor’s Degree in a human services field with 1 year of related experience.
- QMHP-A certification.
- Strong writing and case conceptualization skills with the ability to develop effective treatment interventions tailored to mental health and substance recovery needs.
Additional Qualifications:
- Valid driver’s license and automobile for client transport with at least state minimum insurance coverage.
- Must have and maintain a working cell phone.
