- Collaborates with Team Leader or designee to effectively and efficiently prioritize visit content and plan visits based on client capabilities, condition, preferences (when possible), physician orders, and available resources.
- Maintains a working knowledge of conditions of participation, applicable federal, state, and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures, adhering in a manner that reflects honest, ethical, and professional behavior.
- Accurately assesses clients at admission and other required time points, utilizing appropriate standardized documentation. Determines the agency’s ability to meet client needs in the home care setting.
- If therapy-only case, informs the client/caregiver about the agency, care/services available, client obligation for payment, and rights and responsibilities before care is initiated.
- Develops an appropriate assessment-based plan of treatment with the physician and client. The plan includes attainable episode goals and agency services needed to improve and/or stabilize the client’s medical condition and promote independence. Participates in timely revision of the plan of treatment as appropriate.
- Makes appropriate referrals for evaluation/care to other disciplines and services and coordinates care with others to ensure effective and efficient care delivery.
- Utilizes interview, observation, and evaluation in assessing clients and applies professional judgment, consistent with practice standards, in formulating therapy interventions and making recommendations to the physician and client.
- Reports changes in client condition appropriately and in a timely manner to the client’s physician and obtains orders for changes in the plan of treatment.
- Initiates and implements appropriate preventive, therapeutic, and rehabilitative procedures and techniques for clients.
- Performs manual therapeutic exercises, massage, and/or manual traction as established by the plan of care.
- Evaluates, administers, and adjusts the application of physical agent modalities.
- Instructs, motivates, and assists clients in non-manual exercises and functional activities.
- Evaluates, fits, and adjusts prosthetic and orthotic devices and recommends modifications to prosthetists/orthotists.
- Advises, consults, and, when appropriate, instructs clients, family, or other agency personnel in the client’s therapy program, rehabilitative and restorative care, and ensures evidence of discharge (DC) planning.
- Utilizes and implements appropriate tools and pathways per guidelines and/or policy and procedure (e.g., education checklists, pathway protocols, Home Program Instructions, etc.).
- Participates in care conferences, providing updated assessments, recommending potential revisions to the client’s plan of care, and facilitating achievement of top-decile outcomes and operational efficiencies.
- Participates in quality improvement processes, including peer review, as directed.
- Demonstrates a proactive approach to problem-solving any client care issues.
- Evaluates the home environment and makes appropriate recommendations.
- Submits/transmits documentation in a timely manner as required by agency policy/practice standards. Utilizes point-of-care documentation and documents in the home.
- Performs other duties as assigned to meet business demands.
- Minimum Qualifications
- Possession of a degree, which includes an internship from a baccalaureate/university program in physical
- Licensed to practice as a physical therapist in the state of practice.
- Must have a minimum of one (1) year experience as a physical therapist or have a preceptor for a minimum
- Current working knowledge of conditions of participation and home healthcare experience preferred.
- Must accept responsibility for maintaining clinical practice skills, learning and adhering to organization’s
- Must be able to function in an environment with minimal direct supervision, accepting personal responsibility
- Must have current Driver’s license and reliable transportation to and from work site.
- Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and
