Position Summary: Home Care Coordinator
The Home Care Coordinator plays a crucial role in facilitating referrals to Saint Joseph VNA Home Care from Saint Joseph Regional Medical Center in Mishawaka. This position involves educating acute care staff, including physicians, about the services offered by Saint Joseph VNA Home Care. Additionally, the coordinator refines and improves transition processes from hospital to home care and may participate in business development activities related to expansion or new program development.

- RN/LPN or MSW required
- Bachelor’s degree or equivalent experience
- Graduate of an approved clinical education program and current clinical license in the state of employment preferred
- Minimum of two years' experience in an acute care setting, customer service, sales/marketing role, or relevant experience
- Valid driver’s license and reliable transportation
- Commitment to the mission and Organizational Code of Ethics, adherence to the Compliance Program
- Strong oral and written communication skills
