Key Responsibilities
- Provide direct patient care under the supervision of a Registered Nurse (RN) or physician, including medication administration, wound care, and monitoring of vital signs.
- Collaborate with healthcare providers to develop and implement individualized care plans based on patient needs and physician recommendations.
- Educate patients and their families on disease management, medication administration, and lifestyle modifications to improve overall health outcomes.
- Perform routine assessments and document patient conditions, treatments, and progress in accordance with healthcare regulations and MercyOne policies.
- Coordinate with the interdisciplinary team to ensure comprehensive, patient-centered care is delivered.
- Monitor and report any changes in patients' health status to the supervising RN or physician promptly.
- Maintain up-to-date knowledge of home health care protocols, infection control practices, and safety guidelines.
- Ensure proper use of medical equipment, supplies, and documentation software.
- Comply with regulatory and MercyOne standards for home health care, including participation in quality assurance and performance improvement programs.
- Assist in coordinating services such as physical therapy, occupational therapy, and social work as needed.
- Active Licensed Practical Nurse (LPN) license in the state of Iowa.
- Minimum of 1 year of clinical nursing experience, preferably in home health or community-based care.
- CPR and Basic Life Support (BLS) certification required.
- Strong interpersonal skills and the ability to work independently in a home setting.
- Knowledge of home health regulations and documentation practices.
- Proficient with electronic health records (EHR) systems and basic computer skills.
- Valid driver’s license and reliable transportation, with willingness to travel to patient homes in the Sioux City area.
- Experience with wound care, medication management, and chronic disease management.
- Familiarity with home health equipment and telehealth technology.
- Experience with geriatric or hospice care.
- Ability to lift, carry, and transfer patients as needed.
- Ability to drive between home visits.
- Must be able to walk, sit, and stand for extended periods during patient visits.
