Description
- Collaborates with physicians and key members of the interdisciplinary team to provide services for the diagnosis and treatment of benign and malignant conditions, from the initial abnormal screening study to treatment, rehabilitation &follow-up care.
- Collaborates with Cancer Program teams and Administration to identify growth opportunities for the Cancer Program.
- Acts as liaison for coordinating patient care including office visits, tests and surgical procedures. Reviews Medication reconciliation with patient when necessary.
- Assists patients in obtaining medical records from both internal and external facilities.
- Educates patients and families.
- Acts as liaison and resource person for all ancillary services including the SMMC Cancer Center and the American Cancer Society
- Identifies patients for clinical trials/ makes referrals to Research Center as appropriate.
- Identifies patients for Risk Assessment Program and Genetic Counseling and coordinates referrals as appropriate.
- Develops and reviews Survivorship Care Plans at end of treatment with patient.
- Bachelor's Degree
- 2 years of Oncology experience required
- PA RN License
- OCN Certification Required
