Position Overview:

This per-diem role involves home visits, virtual sessions, and telephonic outreach.
Key Responsibilities
- Assess participants' social determinants of health using validated tools.
- Reinforce asthma care education, medication adherence, and environmental trigger management.
- Connect participants to local health and social services.
- Conduct environmental assessments in homes using the EPA guidelines.
- Provide resources to mitigate asthma triggers and track progress on environmental changes.
- Refer clients to community services (e.g., food, housing, smoking cessation).
- Document all activities accurately and participate in follow-ups, including phone calls and scheduling.
- Engage in community outreach and collaborate with project teams.
- Strong interpersonal, organizational, and leadership skills.
- Knowledge of community resources and public health systems.
- Ability to work with diverse populations while maintaining confidentiality.
- Self-motivation and teamwork skills.
- Experience in home visits and mobilizing communities.
- High School Diploma or GED.
- Must have at least 1 year of in home visiting experience.
- Minimum 2 years in human services or 1 year with vulnerable populations.
- Valid Connecticut Class 2 driver’s license and personal transportation.
- Certified CT CHW certification is required.
- Availability for occasional evenings and weekends.
