What are the responsibilities and job description for the Nurse Navigator RN - Urology position at Trinity Health Of New England?
Position Purpose:
We’re looking for a compassionate and motivated RN Nurse Navigator to join our team in Hartford, CT and will be supporting our local Men’s Health practices and partnering area hospitals. In this role, you’ll be the go-to guide for patients referred to Urology and other subspecialties, helping them navigate their care with ease and confidence. You’ll coordinate their journey from consultation through treatment, ensuring they—and their families—feel informed, supported, and cared for every step of the way. You’ll also have the opportunity to educate residents, sharing your expertise and helping shape the next generation of healthcare providers.
What you will do:
- Coordinating care for patients among general surgery, surgical oncology, bariatric, minimally invasive, and robotic surgical services among the multi-specially surgeons end providers, in order to streamline comprehensive surgical care for each patient
- Communicates effectively (written and verbal) with colleagues, patients and family members
- Educate patients and family members so that they have a transparent understanding of the care being provided.
- Understanding of computer-based applications (E-MAR, clinical documentation), email, and standard applications such as Microsoft Office.
- Collaborates with required services, including radiology, pathology, laboratory, and primary care & referring offices etc to provide seamless care for patients
Minimum Qualifications:
- Education: Graduate of an accredited School of Nursing. Bachelor of Science in Nursing (BSN) degree
- Licensure: Current licensure as a registered nurse (RN) in Connecticut
- Experience: 3 years of practical experience in surgical nursing and familiarity with new trends and practices in the surgery, including minimally invasive surgery and robotic surgery. Certification: BLS certification required
- Ability to pass drug screenings and background checks
