- Provide assistance to participants and families in initiating Medicaid applications and completing required paperwork.
- Conduct home visits for new enrollees and re-determinations, submitting cases to county technicians within approved time frames.
- Educate participants and families on Medicaid regulations and eligibility.
- Maintain Medicaid eligibility records and ensure all documentation is up-to-date.
- Collaborate with internal teams and external agencies to ensure efficient service delivery.
- High school diploma or equivalent.
- At least 1 year of experience working with the elderly or frail population.
- Comprehensive knowledge of Medicaid systems and benefits.
- Strong written and verbal communication skills.
- Ability to work independently and manage multiple tasks efficiently.
