ONSITE, Full Time
Position Purpose:
As a Social Worker in the PACE (Program of All-Inclusive Care for the Elderly) program, you will plan, organize, and implement social services for participants and their families. This includes assessments, counseling, education, and case management to support the physical, emotional, and social needs of elderly participants.
What You Will Do:
- Provide individual and family counseling, support, and education on aging, dementia, grief, and more.
- Conduct assessments and collaborate with the interdisciplinary team to create comprehensive care plans.
- Offer resources and advocacy for participants and families, addressing mental health, finances, and legal needs.
- Assist with participant intake, eligibility, and disenrollment processes.
- Conduct home visits and coordinate care transitions between facilities and home care.
- Promote collaboration within the team to ensure a seamless participant experience.
Minimal Qualifications:
- Master’s degree in Social Work and 2 years of experience, including work with elderly or frail populations.
- State license in Social Work preferred.
- Experience with Medicare, Medicaid, and other social service systems.
- Valid driver's license and reliable transportation required.
- Strong communication, organizational, and problem-solving skills.
- Ability to work in a collaborative and fast-paced environment.
