Key Responsibilities:
- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify and address the physical, psychosocial, and environmental needs of patients and their families, reassessing as needed every 15 days.
- Develop and implement individualized plans of care, collaborating with patients, families, attending physicians, and other members of the interdisciplinary group (IDG).
- Document patient progress, setting realistic goals, and implementing effective interventions to improve patient outcomes.
- Provide education and guidance to patients and their families on self-care techniques and disease management.
- Maintain accurate and relevant clinical records, detailing patient condition and treatment.
Requirements:
- One year of clinical experience in a healthcare setting.
- Current RN licensure in the state of practice, with one year of clinical experience.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
