Position Summary
The Hospice Liaison is a key member of the Trinity Hospice team, responsible for driving business development and fostering relationships with referral sources. This role will identify patients who may qualify for hospice care, conduct informational meetings with families and facility staff, perform patient screenings, and represent Trinity Hospice in educational and support‑group settings.
Key Responsibilities
- Business Development & Outreach
- Build and maintain strong relationships with Skilled Nursing Facilities (SNFs), long‑term care facilities, hospitals, physician practices, and community agencies.
- Proactively identify and cultivate new referral sources through regular on‑site visits, networking events, and targeted presentations.
- Track outreach activities and referral metrics, reporting outcomes to leadership.
- Patient Identification & Screening
- Review clinical records and work with discharge planners and case managers to identify patients meeting hospice eligibility criteria.
- Collaborate with interdisciplinary team to ensure smooth transitions into hospice care.
- Family & Community Education
- Hold informational sessions for patients, families, facility staff, and community support groups on hospice philosophy, services, and eligibility.
- Develop and deliver engaging presentations on end‑of‑life care, advance directives, and symptom management.
- Provide clear, compassionate education materials and answer questions to empower informed decision‑making.
- EMR & Documentation
- Utilize the organization’s Electronic Medical Record (EMR) system to document all outreach, screenings, and educational activities accurately and promptly.
- Ensure compliance with documentation standards, privacy regulations, and hospice Conditions of Participation.
- Interdisciplinary Collaboration
- Participate in weekly sales and strategy meetings with Business Development and Clinical teams.
- Coordinate with nursing, social work, and clinical leadership to align referral goals and patient care plans.
- Act as the internal advocate for hospice services, ensuring best‑practice handoffs and patient satisfaction.
Qualifications
- Bachelor’s degree
- Minimum of 5 years of experience in hospice care, business development.
- Demonstrated success in developing referral networks and meeting outreach goals.
- Strong clinical knowledge of hospice eligibility criteria and end‑of‑life care standards.
- Proficiency with EMR systems
- Excellent public‑speaking skills and comfort presenting to diverse audiences.
- Exceptional interpersonal and communication skills—able to build rapport with clinicians, patients, and executives.
- Valid driver’s license and ability to travel
