Role Description
This is a full-time hybrid role for a Hospice RN Case Manager/Clinical Manager at Trinity Hospice. The role is located in Falmouth, MA with the flexibility for some work within the office. The RN Case Manager/Clinical Manager will be responsible for providing direct patient care, symptom management, coordinating home care services, and collaborating with the Director on clinical operations.
Qualifications
- Medication and Nursing skills
- Palliative Care and Direct Patient Care skills
- Experience in Home Care
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Current RN license in the state of Massachusetts
- Atleast 2 years of hospcie case management experience (preferred)
