Job Overview : The primary function of the Nurse Practitioner role is to assist in overseeing the clinical care and treatment of our patients and their families. In addition, you will consult with the patients' attending physician, in collaboration with our Medical Director(s). Also, you will provide clinical direction to the interdisciplinary team to assure the highest quality of care.What Does The Nurse Practitioner Do Every Day?Collaborates with the IDT in assuring appropriate evaluation and palliative treatment of terminal disease.Consult with and assist in the IDT's development of patient / family plan of care to assure quality care is provided.Educate the patient / family regarding disease process, self-care techniques, end-of-life care.Provide training to other clinical staff as needed.Perform and document visits to patients / family based on a plan of care.Interact with attending and consulting physicians as necessary regarding palliative care.Actively participate in the interdisciplinary team.Provide clinical assessment results to hospice physicians after making face-to-face encounters with patients.Provides on-call services to patients / families as assigned.Background and Education You Need to Be Successful : Graduate of an accredited School of Nursing programCurrent State license as a Registered Nurse and as an Advanced Practice NurseCurrent Driver's License in the state in Texas.2 years of experience as an APN, in a clinical care setting, with background with a hospice or home health agency highly preferred.Comprehensive knowledge of pharmacology, palliative care, and end of life care.Exceptional communication and interpersonal skillsProof of Hepatitis consent / declination.
