What are the responsibilities and job description for the Registered Nurse Case Manager position at Trinity Hospice?
Hospice RN Case Manager experience for a hospice agency REQUIRED. This Registered Nurse (RN) will need to live in either the South (Pearland area) or North (The Woodlands, Spring, possibly Conroe depending on census).
Primary Focus of the Registered Nurse Role:
In this impactful role, you will administer high quality nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary hospice group, patient, family and referring agency; and assume the responsibility for coordination of care of our patients.
What You Will Be Doing Every Day:
Every day, you will be working in our patient’s homes offering up the highest quality nursing care. You will apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility. In addition, you will develop & implement a plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.
Furthermore, you will determine scope and frequency of services needed based on the acuity of your patient and their needs, and their family. Also, you will assess the ability of other members of the care team to meet our patient’s immediate needs upon admission and throughout care.
We take infection control extremely seriously, so you will apply concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
Finally, in this role, you’ll be exposed to blood and bodily fluids and infections diseases. You will need the ability to work a flexible schedule, to travel locally, exposure to unpleasant weather, and the ability to take emergency calls on a PRN basis.
The Background & Education You Will Need To Be Successful:
You will need to be a graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing and hold a current registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC) in the state in which you will practice. In collaboration with this education and licensure background, you will need at least two years of experience as a Registered Nurse (RN) in a clinical care setting preferred, with experience in the hospice space required.
Some of those soft skills you will need to be successful include exceptional nursing skills as defined as generally accepted standards of practice, and excellent interpersonal skills. Experience and specialized skills preferred in palliation and end-of-life care. Finally, you will need to be able to read, write and comprehend English.
Some other tasks you will complete every day include:
- Provide clinical directions to the Hospice Aide and LPN/LVN to ensure quality and continuity of service provided.
- Prepare clinical and progress notes that demonstrate progress toward established goals. Ensure continuity of quality patient care delivered with appropriate documentation.
- Inform physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient and family response to care.
- Monitor assigned cases to ensure compliance with requirements of third party payor.
- Perform on-call responsibilities and provide on-call service to patients and families as assigned.
The Other Pieces You Should Know:
You will need reliable transportation, a current Drivers’ License in the state in which you will practice with valid and current auto liability insurance (we’ll cover your mileage!), and a valid CPR certification.
