We are seeking a Registered Nurse (RN) to join our team to perform client assessments, create personalized care plans, and support our caregivers in providing exceptional service.
Key Responsibilities:
Conduct initial in-home assessments for older adults and veterans (VA program) to evaluate care needs.
Develop and document individualized care plans tailored to each client’s needs and preferences.
Provide ongoing reassessments and updates to care plans to reflect any changes in clients’ conditions.
Serve as a resource and liaison for clients, families, caregivers, and healthcare providers.
Support caregivers in understanding and implementing care plans to ensure high-quality service delivery.
Educate clients and families about available resources, care expectations, and health maintenance strategies.
Maintain professional, respectful, and ethical interactions with clients, families, and team members.
