About the Role
As the Intake Manager, you will be the first point of contact for potential clients and their families. Your role is crucial in understanding their needs, guiding them through our services, and ensuring a seamless onboarding process. You will work closely with our care coordination and operations teams to provide an exceptional experience from the very first interaction.
Key Responsibilities
-Handle inbound inquiries from prospective clients, families, and referral sources via phone, email, and online requests
- Conduct initial consultations to assess client needs and match them with appropriate services
-Educate clients and families about Trinity In Home Care’s services, pricing, and care process
- Manage the intake process, including gathering documentation and ensuring a smooth transition to care coordination
- Collaborate with caregivers, scheduling, and management teams to ensure timely service delivery
- Maintain accurate records and client information in our database
- Build relationships with community partners, hospitals, and referral sources
