Hospice Director of Nursing

JOB RESPONSIBILITIES :

As a Director of Hospice you will be responsible for the overall direction of all hospice clinical services and report directly to the Administrator / CEO. The Director of Hospice establishes, implements, and evaluates goals and objectives for hospice services that meet and promote exceptional quality of care. This position supervises the Hospice Community Nurse, Community LPN, Social Workers, Chaplains, Volunteer Coordinator, Nurse Practitioners, Community On-Call Nurse, Home Health Aides, and Hospice Lead.

RESPONSIBILITIES :

Coordinates and oversees all direct and indirect patient services provided by the hospice clinical team and ensures that patients’ plans-of-care are developed, implemented, and evaluated. Provides twenty-four (24) hours per day, seven (7) days / week on-call coverage.

Provides guidance and counseling to the hospice team to assist them in continually improving all aspects of hospice care services.

Provides help in assessment, planning implementation, and evaluation of patient and family / caregiver care to all clinical personnel as indicated.

Works with the Administrator and the Chief Financial Officer in the preparation and administration of the Agency’s budget in identifying budgetary requirements and determining appropriate use of allocations.

Works with the Administrator / CEO and Human Resources and in coordination is responsible for recruiting, hiring, evaluating, and terminating service personnel.

Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues, or expenses.

Oversees the maintenance of patient clinical records, statistics, reports, and records for purposes of evaluation and reporting of program activities.

Assures proper maintenance of clinical records in compliance with local, state, and federal laws.

Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.

In coordination with the Clinical Educator, plans and implements in-service and continuing education programs to meet education and training needs of Agency personnel.

Assists with the evaluation of Agency performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided by the Agency.

Assists in the development of Agency goals. Develops, recommends, and administers Agency policies and procedures and ensures compliance with federal, state and local rules, accreditation standards and hospice policies in all patient care aspects of hospice.

Assures compliance with all local state and federal laws regarding licensure and certification of Agency staff.

Stays informed about changes in the field of nursing and hospice care; shares information with appropriate organization personnel.

Plans and arranges for consultation needs of staff; prepares and maintains current policies and procedures which meet Medicare, Medicaid, accreditation standards, state and federal laws, etc. and implements; revises concurrently.

Assures physician approval of plans for continuity of medical care.

Investigates and reports any problem relating to patient care or conditions which might harm the patient and / or employee well-being. Immediately reports any accident, incident, lost articles or unusual occurrence to the Administrator.

In coordination with the Designated Officer of Operational Excellence oversees Hospice's ongoing Quality Assessment / Performance Improvement (QAPI) program.

With the Hospice Lead (Where appropriate) coordinates the Interdisciplinary Team Meeting.

Other duties as assigned by the Administrator / CEO.

QUALIFICATIONS :

Registered nurse in the State of Texas with current licensure to practice professional nursing in the State.

Bachelor’s degree in Nursing from an accredited program by the National League for Nursing preferred. Associate degree Nursing.

Three years of management experience in a hospice preferred or related health care organization.

Demonstrated ability to supervise and direct professional and administrative personnel.

Ability to market and deal tactfully with customers and the community.

Possess excellent observation, verbal, and written communication skills.

Knowledge of business management, governmental regulations.

Must have a current valid driver’s license, auto liability insurance and reliable, insured transportation.
