POSITION OVERVIEW
The "Residential Supervisor," formally known as Team Leader, is under the direction and supervision of the Network Director, the Team Leader supervises and guides all individual community and home activities. The Team Leader supervises all staff assigned to the home, participates as a member of the Community Support Team in the development and completion of assigned implementation strategies (flourish plans), and represents the organization in leadership and administrative activities. The Team Leader also ensures a positive home environment. A critical component of the Team Leader's role is to ensure the safety and wellbeing of individuals served in order for all to live full and abundant lives.

Essential Functions

Provides complete oversight and supervision for two or more twenty-four (24) hour or intermittent community residential sites, which includes on-call responsibilities and, at minimum, two late nights per work week and one weekend per month; each site requires supervision to up to two (2) or more full-time Independent Living Coaches (ILC).
Reviews Independent Living Coach applications and interviews potential candidates for the Independent Living Coach position.
Provides recommendations to the Network Director regarding hiring, onboarding including on-the-job trainings (OJTs) & Quest, firing, promoting, and evaluation of Independent Living Coaches within the supervised homes.
Assigns work schedules or rotations for Independent Living Coaches assigned to the home.
Delegates responsibilities to two (2) or more full-time Independent Living Coaches.
Delivers quality onboarding to newly hired Independent Living Coaches which includes orientations, on-the-job trainings (OTJs) and Quest within the first few months of the new employees' employment.
Delivers coaching, and leadership to the team or teams of Independent Living Coaches assigned to the supervised home(s).
Instructs Independent Living Coaches in the proper methods of conducting individualized programs and activities.
Communicates vital information to staff including updates and policy changes during routine staff meetings; provides meeting minutes to the Network Director upon completion of the meeting.
Communicates with parents, guardians, staff, visitors, community agencies, and other internal or external entities in a professional and timely manner.
Functions as a member of the Trinity leadership team in the organization including task groups; is actively involved in the network and organization-wide committees.
  • Works with other Trinity staff members in developing and suggesting events and system improvements.
  • Responsible for ensuring a clean and safe environment for program participants and employees; which includes completing necessary checklists and initiating any necessary maintenance requests.
  • Participates with the Network Director and other executive committee members in the development of the home budget and related financial business.
  • Recommends cost-saving techniques for the supervised residential site(s), which may include budgeting and purchasing of groceries and household supplies at the supervised residential site(s).
  • Responsible for initiating repair and maintenance requests by conducting regular home and environmental checks.
  • Provides complete oversight and monitoring of electronic and paper documentation.
  • Helps program participants with their priority outcomes identified in their person centered plan.
Instructs and carries out written implementation strategies and behavior support plans.
  • Encourages and monitors individuals to engage in community activities and events, individually and on a small group basis.
  • Promote natural support networks to facilitate continuity in existing relationships and build new relationships.
  • Advises the Network Director and the Network Nurse (or designee) of any immediate or non-immediate medical problems regarding any program participants.
  • Monitors the implementation of medical and special dietary needs of each program participant across all assigned home(s).
  • Monitors the oversight of medication administration across assigned home(s).
Works collaboratively with the staff at community day services and/or Supportive Employment in seeking growth for program participants.
  • Provides ongoing monitoring of program participants needs including physical health, emotional wellbeing and preferences.
  • May provide or arrange for transportation of program participants at the supervised residential site(s) to and from recreational, social, work or similar activities and medical or other appointments.
  • Ensures staff attends and maintains all required trainings (this may include but not limited to medication administration training, CPR, First Aid and Safety Care).
  • Other duties as assigned.

  • Able to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, necessary for providing quality care
  • Able to communicate policies, procedures, regulations, reports, etc. to staff, program participants, family members, visitors, and government agencies/personnel
  • Able to seek out new methods and systems and be willing to incorporate them into existing practices
  • Excellent oral and written communication skills
  • Excellent interpersonal skills
  • Strongly self-motivated

Physical Demands

  • Be able to lift over 40 pounds from floor to waist unassisted
  • Be able to push and pull 75 pounds
  • Be able to complete deep knee squats
  • Be able to push wheelchairs up to 200 pounds or more
  • Remain in a stationary position, often standing or sitting for prolonged periods
  • Operate motor vehicles
  • While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach, stoop kneel to install computer equipment
  • Physical activities that could be used include: driving, balancing, pulling, pushing, standing, climbing, climbing up and down stairs, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, and repetitive motion.

Qualification

  • High School diploma or GED and
  • Valid Drivers License
