What are the responsibilities and job description for the Paraprofessional - Direct Care position at Trinity Social Services?
Provide direct care (Case Management, Psychosocial Rehabilitation, CSI, Family Skills, or other Authorized Services). As a Trinity Paraprofessional, you will support students individually or in small groups in one of the following areas: Autistic, Emotional, Instructional, Learning, and Life Skills and much more.
Minimum Qualifications:
- High school diploma/equivalent OR Bachelor’s degree from an accredited college or university. AND
- Completion of standard training requirement for paraprofessional from Relias Essential Learning (includes training provided by the organization and online training).
- Under supervision of an appropriately licensed/credentialed professional at TSS.
- Person with a minimum of 1-year experience working with clientele suffering from MH/SA issues and their families is a plus.
- Ability to connect with students and motivate them in a positive way.
- Ability to communicate with co-workers and business contacts in a courteous and professional manner.
- Strong organizational skills and attention to detail.
- Ability to work independently and coordinate multiple tasks simultaneously with minimal supervision.
- Ability to exercise reasonable judgment and maintain confidentiality.
Duties Include:
- Provide direct care (Case Management, Psychosocial Rehabilitation, CSI, Family Skills, or other Authorized Services)
- Responsible for staffing cases when assigned, when requested, and when you believe necessary.
- Provide designated services as dictated by active/current treatment plan of the individual in treatment.
- Render crisis protocol, as needed.
- Be expected to transport your consumer to a yearly psychological or other appointment that your consumer is required to complete to continue services, according to the most recent FY Medicaid guidelines, policies and procedures, and TSS guidelines, policies, and procedures
- Submit complete deliverables (documentation- BIRP note, non-billable, discharge summaries, reauthorization forms, invoices, etc.) within 48 to 72 hours or as requested of project.
- Communicate with supervisory staff, when needed, ensuring client care and clinician concerns are known.
- Maintain monthly clinical supervision (non-licensed clinician).
- Keep all required trainings, certifications, and other critical documents current (i.e. CPR/FA, CPI, Fingerprint Background Check, Professional Liability, etc.).

- Communicate effectively, clearly, and appropriately and ensure client care and clinician concerns are known.

- Prompt arrival to sessions/meetings is strongly encouraged.
