What are the responsibilities and job description for the Licensed Practical Nurse (LPN) - AL position at Trinity Woods?
Days - 7a-7p - Thursday - Saturday and every other Wednesday in Memory Care Assisted Living
Job Requirements:
- High school graduate or GED preferred.
- Minimum of two (2) years work experience as a nurse in a long-term care environment preferred.
- Meets all state requirements for licensure as a practical nurse and Food Handling.
- Commitment to person-centered and resident-directed care is essential.
- Must have knowledge of federal and state regulations governing clinical care and nursing practice in long-term care.
- Must have demonstrated leadership qualities, supervisory skills, and interpersonal relationship skills.
- Completion of cross training for household activities, household housekeeping, household laundry and household homemaking, including food service and food handling required.
Responsibilities:
Meets all resident and household needs by actively assisting (to the extent of cross training and certification) in all tasks to meet identified needs.
- Clinical
- Ensures the clinical operations in the household are in compliance with federal, state, and local regulations while honoring person-centered and resident-directed care.
- Provides leadership to household staff to maintain policies and procedures for clinical practices and household management that support the values of person-centered care and resident-directed care.
- Implements clinical policies and procedures which ensure safe, sanitary and efficient practices supporting professional standards of nursing care.
- Ensures that documentation, medication administration, resident nursing care, safety and sanitation, staff development and resident rights are in compliance with facility standards.
- Fosters collaboration, consensus and negotiation to address problem solving in the household; assures all clinical policies and procedures of the household are maintained; interprets and communicates same to all in the household as needed with emphasis on resident, family or designated representative relationships.
- Maintains working relationship with physicians and other medical personnel in support of quality clinical care in the household.
- Consistently works with all household team members, led by resident and family preference to meet the needs and desires of all household residents.
- Dining Services
- Helps to prepare and serve appropriate snacks and meals. Requires certification in food handling.
- Assists elders who are unable to eat without support as appropriate.
- Facilitates mealtime conversations and maintains a warm, pleasant atmosphere while elders are eating.
- Assists with kitchen-related activities for the residents.
- Life Enhancement
- Assists with the planning and organizing of social activities for individual residents, small groups and for the household.
- Continually seeks opportunities to support the social needs of all members of the household through support, friendship and kindness.
- Seeks opportunities throughout the day to facilitate spontaneous activities of resident choice and household preference; accompanies residents in all activities of choice.
- Continually seeks opportunities to share person interests and talents with members of the household through both planned and spontaneous activities.
- Housekeeping
- Maintains clean spaces in the household, including resident rooms and household common spaces, as well as support spaces and equipment.
- Maintains supply of all materials to meet resident needs in the household.
- Coordinates work with the household homemaker to ensure the housekeeping needs of the household are met within the staffing resources of the household.
- Laundry
- Provides assistance to ensure residents’ personal laundry is maintained in quality condition.
