Position Description
Health Services Coordinator (HSC)
REPORTS TO: Administrator
SUPERVISES: Activity Assistant, Activity Volunteers
POSITION: Health Services Coordinators (HSCs) work in healthcare settings, and their primary responsibility is to support the healthcare team in organizing and coordinating clinical activities. Their duties involve maintaining MDS, medical records, scheduling, and being the first point of contact for patients and visitors.
POSITION QUALIFICATIONS:
- Experience: Previous MDS experience preferred. Previous leadership experience preferred. Must have RN license. Knowledge of healthcare setting, or similar environment preferred.
- Skills: Strong interpersonal and communication skills; ability to work effectively with diverse populations.
- Empathy: Compassionate and patient approach when dealing with residents and families.
- Organizational Skills: Ability to prioritize tasks and manage time effectively in a fast-paced environment.
- Problem-Solving: Strong critical thinking skills and the ability to handle challenging situations calmly and effectively.
- Computer Skills: Basic proficiency in Microsoft Office and experience with electronic health records (EHR) and PCC is a required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Resident Support: Assist residents with emotional, social, and psychological needs, providing a listening ear and guidance. Assists with financial and community help as needed.
- Assessment: Conduct initial and continued assessments to identify residents' social service, clinical, and activity needs creating care plans in collaboration with the interdisciplinary team. Sets up MDS schedules.
- Resource Coordination: Connect residents and families with community resources, services, and support networks.
- Care Plan Implementation: Collaborate with the care team to implement and monitor individualized care plans, ensuring that social service, clinical, activity and level II PASRR needs are integrated into overall care.
- Family Engagement: Facilitate communication between residents, families, and staff, ensuring families are informed and involved in care decisions.
- Crisis Intervention: Respond to resident crises, providing support and intervention as necessary, and escalate issues to the appropriate staff when needed.
- Documentation: Maintain accurate and timely documentation of social service, and clinical interactions, assessments, and care plans in compliance with facility policies and regulations.
- Advocacy: Serve as an advocate for residents’ rights and needs, promoting dignity and respect in all interactions.
- Education and Training: Provide information and training to staff and families regarding social services and community resources.
- Compliance: Ensure all services comply with local, state, and federal regulations and guidelines.
Admissions and Discharges
· Coordinate with provider to ensure referrals are complete
· Confirm payer source for new admission and receive a pre-authorization from insurance as needed
· Prepare Point Click Care with new resident information
· Coordinate safe effective discharges
· IPOSTs are complete and up to date
· ABNs, Nomnocs, SNF re-certs completed timely
· Bed Holds
PASRR
· Acquire or create PASRR prior to admission
· Update PASRR’s and path tracker as needed
· Completes all sections of MDS, assessments, and UDA’s for admissions, significant changes, quarterlies, and annuals.
· Psychotropic drug monitoring, consents up to date.
Care Conferences
· Schedules and runs Care Conferences with family members and residents. Notifying Interdisciplinary Care Conference team of dates and times.
· Create care plans with consistent updates on level 2 PASRR, Psychotropics, and other psychosocial or clinical needs
Human Resources
· Creates and monitors schedule for nursing department in a cost-effective manner as needed.
· Interprets facility policy and procedures, rules and regulations to subordinate nursing staff, residents, families and appropriate medical and ancillary personnel.
· Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department.
· Report unsolved complaints and grievances to the Administrator.
Activities
· Able and willing to get Activity Director Certificate
· Oversee activity department ensuring all residents are receiving needed activities; and documentation reflex this.
· Steps in or finds another to complete activities if the assistant is not able
· Facilitates resident council
Transportation
· Schedule and verify transportation to medical appointments via Medicaid transport if facility is not able to transfer.
Administrative
· In the event the administrator is out of office
o Complete office duties as assigned as stated but not limited to opening mail, sending invoices, mailing out statements and checks, ect.
Budget
· Maintains monthly budget by Administrator discretion.
Regulatory
· Maintains deficiency free outcome from Department of Inspections and Appeals survey.
Safety and Sanitation
· Demonstrates knowledge of emergency policies and procedures
· Performs all duties in a safe and efficient manner; assures that safety regulations are followed at all times by all staff.
Customer Service
· Interacts with administrator, residents, families, staff, volunteers and other visitors to the facility in a pleasant, respectful, and courteous manner.
· Acts as a positive representative of the facility at all times.
· Performs incidental housekeeping and maintenance tasks as may arise during the course of regular duties, in order to maintain a clean, safe, pleasant environment for residents, visitors and staff.
· Responds to any and all call lights within 15 minutes as applicable.
Professional Duties
· Represents Tripoli Nursing & Rehab in a professional manner within the community.
Residents/Patients’ Rights
· Complete HIPPA training yearly and follow all HIPAA guidelines
· Provides privacy and maintains the confidentiality of all resident and staff information.
· Reports all complaints and grievances made by residents, family members, and visitors to the Administrator.
· Ensures that residents are free from abuse (physical, mental, and sexual), mistreatment and neglect, and reports any such instances to appropriate state agencies.
In-Service/Education
· Assists in coordinating and planning monthly in-service training.
· Assigns and monitors CE Solutions and other necessary educations for all staff.
· Monitors that staff certifications/license are up-to date and in good standing.
· Completes personal CE Solution educations in a timely manner.
· Ensures that all staff competencies are completed.
· Attends all in-service education programs mandated by federal, state, and company guidelines.
· Participates and assists in departmental meetings and projects as assigned.
Attendance
· This position works 32-40 hours Monday – Friday unless approved for paid time off. Is accountable for his/her time and work responsibilities.
· In facility work only.
· Attends morning management meetings, unless on preapproved time off.
· Attends Medicare meetings weekly.
· Observes guidelines as established by facility personnel handbook.
Miscellaneous
· All other duties assigned by the administrator.
· Must clock in and out using the all-employee time clock.
· May need to work as floor nurse or RN coverage as needed.
Job Types: Full-time, Permanent
Pay: $38.00 - $42.00 per hour
