What are the responsibilities and job description for the Case Manager position at TriSource?
TriSource has an opening for a Shelter Case Manager that will work with single and family households residing in a shelter in Loudoun County! Day to day responsibilities include: conducting individual/family needs assessments, assisting clients in accessing resources and benefits, conducting goal setting, budgeting, and planning activities with clients and empowering clients’ progress gaining housing. Once housing is secured, the Case Manager will work to link households with services and resources to help them stabilize in their new housing and neighborhood.
Schedule: Monday-Friday 9am-5pm
Duties include but are not limited to:
- Provide relational and responsive case management support to each household
- Develop Housing First focused service plans, which are individualized to the household’s unique housing need; assist each household through the housing location process
- Complete any relevant assessments or referrals needed to support the client’s housing plan and needs
- Once housing is secured, partner with clients to outline what supports are needed to address housing barriers and needs, and to develop an agreed upon plan to achieve housing related goals and to maintain housing stability
- Assist clients in securing needed or requested basic needs items and services such as clothing, employment opportunities, benefits, health resources, employment, childcare, educational resources, etc.
- Assure that all relevant information pertaining to clients is disseminated to all appropriate staff members in a timely manner
Requirements:
- Bachelors degree and at least 1 years experience in human service case management
- Valid drivers license and clean background
- Negative TB/PPD
- Experience in housing case management
Starting pay rate: $25/HR
Job Types: Full-time, Contract
Pay: $25.00 - $27.00 per hour
