Job Summary and Qualifications
The Case Manager RN Wound Care ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.
This is a hands on position. Part time 20 / hrs. a week. The nurse will be primarily leading our inpatient program and rounding on our inpatient wound care consults, assisting and determining appropriate dressings, educating nurses on the inpatient units, etc.. Would also be in the outpatient clinic some.
