Responsibilities

- Oversee patient case coordination & care plans
- Review & audit EMR charts for accuracy & compliance
- Collaborate with clinicians & support staff
- Deliver top-tier customer service to patients & families
Skills
- Unrestricted RN or LVN license in California
- Minimum 1 year of home health experience
- EMR & documentation experience
- Strong leadership & customer service skills
