The successful candidate will be hired as either a Program Coordinator or Program Coordinator Assistant based on their education and qualifications. This position offers an exciting opportunity to support and enhance our programs through coordination, communication, and thoughtful planning.
The Program Coordinator or Program Coordinator Assistant plays a key role in supporting the development and coordination of services for individuals served by Triumph, Inc. Depending on qualifications and education level, the candidate will be hired as either a Program Coordinator or a Program Coordinator Assistant. In either capacity, the role focuses on implementing and maintaining person-centered service plans that align with Triumph’s mission. The Program Coordinator Assistant will support Triumph's Program Coordinator in administrative and day-to-day tasks, while the Program Coordinator will take a lead role in service planning, coordination, and oversight. Both positions are integral to ensuring high-quality, individualized care and fostering a supportive environment for all individuals served.
Summary of essential job functions for either role-
- Ensure all aspects of services provided to all supported individuals meet state and federal regulations.
- Coordinate and facilitate each assigned individuals’ team meetings and communications.
- Maintain regular communication with each assigned individual, their family or guardian, Team Lead and all the individual’s team members as deemed appropriate.
- Support the individual in identifying personal outcomes and supports needed in a person-centered process.
- Develop, implement, and monitor the Overall Service Plans and assessments needed for each assigned individual within the required timelines.
- Work directly with the individuals supported to learn what is most important to them and to provide supports as needed.
- Ensure all plans clearly detail supports needed to attain desired outcomes.
- Monitor and report any progress, changes, or individual needs to the assigned individuals’ team.
- Researching, promoting new activities, opportunities, and adaptive equipment and technology that will enhance and expand services provided.
- Review all documentation pertaining to the individuals’ services completed by support staff (i.e. incident reports, medication administration records, communication logs)
- Adhere and comply with all reporting requirements of abuse, neglect, mistreatment and exploitation.
- Report and maintain benefit records as needed for each assigned individual.
- Utilize Therap programming for developing plans, reporting and other sources of information as needed.
- Maintain accurate documentation records to report to Supervisors as needed.
- Provide Quality Assurance for the agency and those supported.
- Participate and assist with CQL Accreditation. Attend Basic Assurances Committee meetings, communication with CQL representatives, assist in compiling and maintaining required paperwork and follow up from CQL.
- Enforce and support Triumph, Inc. policies and procedures.
- Perform related duties as required.
Minimum requirements-
- A Bachelor’s degree in Human Services, Social Work, Psychology, Gerontology, or a related field is required for the Program Coordinator role. However, a bachelor’s degree is not required for the Program Coordinator Assistant, making this an excellent opportunity for individuals with relevant experience or those pursuing a career in the human services field.

General hours for this position will be Monday thru Friday, 8:00am to 4:00pm. Weekend and evening hours may be required to meet the needs of the individual served. Starting wage depends on the level of education and position hired for. Candidate is eligible for a 50-cent increase after 90 days as long as a favorable performance is established and training requirements fulfilled.
