Discharge Planner – Make a Lasting Impact!

Help clients transition successfully from treatment to independent recovery. As a Discharge Planner , you'll create personalized aftercare plans, connect clients with vital resources, and ensure they have the support they need for long-term success. Your work will involve coordinating care, advocating for clients, and collaborating with clinical teams to make a real difference in their recovery journey.

Ensuring Safety Excellence : In compliance with Washington State's ESSB 5123, we prioritize safety in our workforce by conducting controlled substance testing, including marijuana, for this position, which is deemed safety-sensitive.”

Position Summary : The Discharge Coordinator is responsible for developing and implementing comprehensive discharge plans for clients completing residential treatment for substance use disorders. This role ensures continuity of care by coordinating post-treatment services, including outpatient therapy, sober living arrangements, aftercare programs, and community resources. The Discharge Coordinator works closely with clients, families, treatment teams, and external providers to facilitate a smooth transition and reduce the risk of relapse.

  • RESPONSIBLITIES

    Discharge Planning & Coordination

  • Develop individualized discharge plans in collaboration with clients, clinical teams, and external providers.
  • Assess client needs for ongoing care, including mental health services, housing, employment, and peer support.
  • Coordinate referrals to outpatient treatment, sober living facilities, support groups, and other recovery resources.
  • Ensure clients have a structured aftercare plan, including relapse prevention strategies and follow-up appointments.
  • Client Support & Advocacy

  • Educate clients and families on available post-treatment resources and recovery options.
  • Assist clients with applications for housing, employment programs, Medicaid, or other benefits as needed.
  • Provide crisis intervention or additional support if discharge plans need adjustments.
  • Collaboration & Communication

  • Work closely with therapists, case managers, medical staff, and external agencies to ensure seamless transitions.
  • Maintain strong relationships with community organizations and service providers to enhance referral options.
  • Communicate discharge plans effectively with clients and their support networks.
  • Documentation & Compliance

  • Maintain accurate and timely documentation of discharge planning activities in client records.
  • Ensure compliance with facility policies, licensing requirements, and confidentiality regulations (HIPAA).
  • Participate in team meetings and provide updates on discharge planning progress.
  • QUALIFICATIONS AND REQUIREMENTS

    Education :

  • Bachelor’s degree in social work, Psychology, Counseling, or a related field required
  • Experience :

  • At least one year of experience in case management, discharge planning, or substance use disorder treatment preferred
  • Strong knowledge of SUD treatment and recovery resources.
  • Excellent communication and organizational skills.
  • Ability to manage multiple cases and work collaboratively with a multidisciplinary team.
  • Familiarity with HIPAA regulations and client confidentiality requirements.
  • Familiarity with 42 CFR part 2 preferred
  • Licenses / Certificates / Registration :

  • Valid drivers license and insurance. A minimum of 5 years’ licensed driving experience.

  • This position requires standing, walking, and bending for long periods.

  • This position may require exerting up to 40 pounds of force to lift, carry, pull, or move objects
