Discharge Planner – Make a Lasting Impact!
Help clients transition successfully from treatment to independent recovery. As a Discharge Planner , you'll create personalized aftercare plans, connect clients with vital resources, and ensure they have the support they need for long-term success. Your work will involve coordinating care, advocating for clients, and collaborating with clinical teams to make a real difference in their recovery journey.
Ensuring Safety Excellence : In compliance with Washington State's ESSB 5123, we prioritize safety in our workforce by conducting controlled substance testing, including marijuana, for this position, which is deemed safety-sensitive.”
Position Summary : The Discharge Coordinator is responsible for developing and implementing comprehensive discharge plans for clients completing residential treatment for substance use disorders. This role ensures continuity of care by coordinating post-treatment services, including outpatient therapy, sober living arrangements, aftercare programs, and community resources. The Discharge Coordinator works closely with clients, families, treatment teams, and external providers to facilitate a smooth transition and reduce the risk of relapse.
