Your Impactful Role as a Case Manager Team Lead:
In your pivotal role based at our Twin Cities office, you will split your time between managing a caseload of clients (50%) and overseeing the training and clinical compliance of our team of RN Case Managers. This position ensures the delivery of high-quality care by focusing on clinical standards, providing ongoing education, and supporting RN Case Managers in meeting compliance requirements.
- Assist in supervision of skilled nurses, participate in hiring and performance management of the clinical staff. Assist with assigning and monitoring the RN Case Manager's caseload and productivity.
- Perform quality review of all nursing documentation, including assessments, visit notes, and other supporting documentation. You'll provide clinical oversight to ensure proper documentation exists to support reimbursement/billing and collaborate with the Start of Care clinician to review key Oasis questions and determine appropriate plan of care.
- Oversee value-based purchasing: and 5-star ratings for assigned region.
