Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality home health care to patients in their residences. Duties
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed, ensuring adherence to aseptic techniques.
- Monitor patient progress and adjust care plans as necessary based on clinical findings.
- Provide education and support to patients and their families regarding health management, including family planning and DME (Durable Medical Equipment) usage.
- Collaborate with interdisciplinary teams in various settings, including inpatient facilities, ambulatory surgery centers, and home environments.
- Document patient information accurately using electronic health records systems such as eClinicalWorks.
- Maintain compliance with healthcare regulations and standards, including ICD-10 coding for documentation purposes.
- Offer specialized care for children when necessary, ensuring a family-centered approach.
Requirements
- Valid Registered Nurse (RN) license in the state of practice.
- Experience in home health or community nursing preferred.
- Proficiency in eClinicalWorks or similar electronic health record systems is desirable.
- Knowledge of ICD-9 coding and documentation standards is a plus.
- Familiarity with aseptic techniques and infection control practices.
- Experience working in inpatient settings or ambulatory surgery centers is advantageous.
- Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals.
- Ability to work independently while managing multiple priorities in a dynamic environment.
