JOB TITLE: Nurse Practitioner
REPORTS TO: Chief Medical Officer/Administrative Manager/Medical Director

About the Role:
The Family Nurse Practitioner (FNP) plays a crucial role in delivering comprehensive healthcare services to patients across various age groups and backgrounds. This position involves assessing, diagnosing, and managing a wide range of health conditions, ensuring that patients receive high-quality care tailored to their individual needs. The FNP will collaborate with a multidisciplinary team to develop treatment plans, provide patient education, and promote wellness within the community. Additionally, the role includes performing procedures such as chemotherapy administration and pain management, while prioritizing patient safety and comfort. Ultimately, the FNP is dedicated to improving patient outcomes and enhancing the overall health of the populations served at TRUE HEALTH locations in Hicksville, Brooklyn, and Great Neck.
Minimum Qualifications:
- Master's degree in Nursing with a specialization as a Family Nurse Practitioner.
- Current and valid Nurse Practitioner license in the state of New York.
- Certification as a Family Nurse Practitioner from a recognized certifying body.
Preferred Qualifications:
- Experience in a primary care or urgent care setting.
- Familiarity with electronic health record (EHR) systems.
- Bilingual skills to effectively communicate with diverse patient populations.
- Minimum 1 year experience as NP
- Graduated from approved Nurse Practitioner Program.
- Licensed as Nurse Practitioner in New York State.
- Basic CPR (BCLS) certification required.
- Specialty Certification Preferred.
- Bilingual in English and Spanish or Creole preferred.
Responsibilities:
- Conduct comprehensive health assessments and physical examinations for patients of all ages.
- Diagnose and manage acute and chronic health conditions, including prescribing medications and treatments.
- Educate patients and their families on health management, disease prevention, and wellness strategies.
- Collaborate with healthcare team members to develop and implement individualized care plans.
- Perform necessary procedures, including chemotherapy administration and urgent care interventions, while ensuring patient safety.
- Involved with direct patient physical assessment, education, and health planning.
- Assists in the coordination of services with other disciplines and patient’s insurance plans.
- Advocates for patient in arrangement of medically necessary services.
- Communicates with community’s resources to ascertain adequate assistance for patients in need.
- Refers patients to specialty services when necessary.
- Consults with collaborating physician and other members of the health care team to establish plan of care.
- Participate in LEAN and other activities necessary to implement the multi-disciplinary patient-centered medical home approach for the care of patients
- Maintains continuity of care by regular patient follow-up.
- Participates in quality assurance process to assure optimal care delivery.
- Provides comprehensive case management services that include emphasis on health promotion and disease prevention.
- Monitors patient outcome in accordance with the practice protocols;
- Collaborates with staff nurses and nursing administrators with assessment and clinical decision in patient care planning.
- Acts as a resource person for the nursing staff.
- Conducts education in service on practice issues.
- Other duties as assigned.
