Job Summary

This position provides outreach and coordinated intake and referral services to pregnant women, interconception women, and infants and their families by providing knowledge, encouragement, linkages, and support necessary to maximize families' health, well-being, and self-sufficiency. This position works to ensure each participant's continued involvement in prenatal and child healthcare as well as other needed community home visiting services. This position will actively engage the participant and build on the families' strengths, assets, and goals. This position conducts and coordinates healthcare services for identified high risk participants in-person via home visits and telephone calls.

Position Expectations And Responsibilities

  • Make telephone calls, home visits, and/or community visits (e.g., clinic settings, hospitals, pregnancy centers) to pregnant women, parents of at-risk infants, and interconception women who have a loss, either through miscarriage/abortion, stillbirth/fetal death, infant death, or have a baby adopted or removed from the home
  • Provide information, education, and encouragement needed to inform and/or motivate families to take steps necessary to change situations placing them at risk
  • Promote self-sufficiency and healthy outcomes through education, encouragement, motivation, reinforcement of health care regimen, anticipatory guidance, supporting home safety, enhancing parent-infant interaction, promoting continuation in health care promoting health literacy, and managing behavior concerns
  • Coordinate client referrals and interagency activities of community resources
  • Facilitate planning and problem solving with participants and families
  • Collaborate with other providers to assure continuity and coordination of care and advocate on behalf of the participant, including communicating to the providers, social workers, networking agencies, and communication of the participant's strengths, needs, and feelings
  • Coordinate service delivery and caseload management in a manner that addresses the immediacy of the participants' needs and identified risks to improve outcomes
  • Accurately code and document services, education, and other pertinent information in participant’s electronic record
  • Conduct intake of client's total medical and social needs through interview and observation
  • Identify, evaluate, and assess in collaboration with families, their strengths, resources, needs, and priorities
  • Establish rapport and develop relationships with families
  • Complete client intake and progress notes accurately and timely
  • Develop and monitor the plan of care to assure that the concerns of families are addressed
  • Follow up with patient on compliance with provided care plan
  • Properly organize client discharge planning and case closure
  • Prepare and submit appropriate administrative report and documents accurately and on a timely basis (e.g., caseload reports, timesheets, logs, etc.)
  • Participate in assigned trainings, supervision, and team meetings as requested
  • Perform all other duties as assigned

Education And Experience

  • A bachelor’s degree or higher from an accredited college or university or higher in social work, social sciences (psychology, human services, biology, sociology) or a health-related field as nursing, health education, health planning or health care administration with two (2) years of public health/community development experience. College transcripts will be required for the successful applicant.
  • BLS CPR Certification required and maintained current

  • Knowledge of methods of compiling, organizing, and analyzing data
  • Knowledge of HIPAA guidelines and maintain confidentiality of all information
  • Must have a valid Florida driver’s license, reliable transportation, and a good driving record
  • Ability to be flexible regarding work location and work schedule, occasionally including evenings or weekends outside of regular business hours, based on business needs.

Preferred Qualifications

  • Bilingual in English and Spanish
  • Prior Healthy Start program experience is preferred but not required
  • Knowledge of referral and community resources processes

Physical Requirements

  • Ability to stand, walk, or view a computer screen for extended periods of time
  • Must be able to sit or stand for an extended amount of time
  • Must be able to perform reaching, lifting, and bending motions
  • Frequent use of keyboard and telephone.
  • Ability to lift up to 25 pounds
  • Ability to perform repetitive hand and wrist motions for extended periods of time
  • Ability to effectively talk and listen to patients

RELATIONSHIP REPORTING

  • Reports to the Program Manager and/or Director of Healthy Start

Healthy Start requires all new hires to pass a local and national Level 2 criminal background check before hiring.
